The parent(s)/guardian(s) of ___________________________ has/have read and agree(s) to abide by the following:

WHEREAS, the undersigned parent(s)/guardian(s) has/have made a decision to enroll my child at the Nap Ford Community School in order to provide a unique educational opportunity of Health and Wellness to my child; and

WHEREAS, I recognize that the Nap Ford Community School is a public charter school of choice not entitlement; and

WHEREAS, my decision to enroll my child at the Nap Ford Community School is based upon my desire to be an active partner in the education of my child.

NOW THEREFORE, in consideration of the foregoing:

As a parent(s)/guardian(s) enrolling my child at the Nap Ford Community School, I agree to support the guidelines adopted by the Board of Trustees by:

• Recognizing that I am the FIRST teacher of my child and embracing my role to have primary responsibility for the education of my child;
   
• Purchasing uniforms for my child from the school's approved supplier and ensuring that my child abides by the dress code of the Nap Ford Community School;
   
• Participating in the five parenting workshops and parent graduation provided by Nap Ford Community School;
   
• Attending all conferences scheduled with any member of the Nap Ford Community School staff;
   
• Participating in the parent volunteer program a minimum of 6 hours per quin. This may include but is not limited to field trips, class visitation, classroom/school presentation, etc.
   
• Providing, each day for my child, a healthy lunch and a snack, either lunch-boxed or purchased from Nap Ford Community School vendor;
   
• Participating in the monthly scheduled Nap Ford Advisory Committee (NFAC). Meetings are scheduled from August to May; and
   
• Encouraging my child to abide by the student code of conduct by supporting the expectations of the code.

IN ORDER to enhance my child's academic, physical, social, emotional and financial Health and Wellness, I agree to do the following:

• To have my child to school on time each day;
   
• To participate in at least one field trip as a chaperone or attend one program during the day;
   
• To read my child's agenda each day and to use the information sent home to keep me informed of school activities and academic topics;
   
• To provide a suitable place and time within my home for homework and to review, sign and return the agenda to the school daily;
   
• To assist my child in obtaining and regularly using a library card at the public library;
   
• To allow for two fifteen (15) minute periods of reading daily;
   
• To limit television and video games during the week and allow more time for reading, studying and family time;
   
• To encourage my child to give his/her best efforts in every task everyday; "Good is not good enough, I will give my BEST!" and
   
• To ensure proper rest at night for my child.

I (WE) understand that not fulfilling my contractual obligations to my child and the school may result in a request to appear before the Nap Ford Community School Board of Trustees.
